Enrolment Information
Hume Valley School provides educational programs for primary and secondary school-aged students (5-18 years) who have a diagnosed Mild Intellectual Disability.
Hume Valley School is not a designated neighbourhood school therefore enrolment is based on our enrolment policy and entry criteria. For your child to be enrolled at Hume Valley School, they must have a diagnosis of Mild Intellectual Disability determined by a psychological assessment which indicates a Full-Scale Intelligence Quotient (FSIQ) between 50-70. If you believe your child meets this criteria please complete the below expression of interest form.
Once a decision to proceed with enrolling your child at Hume Valley School is made, we will provide an enrolment pack (including enrolment form) that will need to be submitted with additional required documents including:
- Evidence of identity and date of birth (for example, birth certificate or passport.
- Immunisation Status Certificate (avaiable from MyGov site)
You will also be required to provide information such as:
- Your contact details
- Emergency contact details
- Legal orders relating to your child (if any)
